by which way we need to pass the username and password to that webservice.

I am having
1. proxy dll file ( creaded from wsdl file)
2. end point url ( https:// endpointurl/test )
3. username and password for that end point url for credential
Please guide me how to procede to make a successfull connection with the webservice.
