What do you mean by server?
Any way you need to configure DNS to access your website by the name.
But if you want using the name in just one machine for testing purposes, you can change the hosts (located on C:\WINDOWS\system32\drivers\etc) file on your test machine and add the following lines in it:

[Server IP Address] www.applicationname.com

for your seconf question:
to set a start up page you can set it on the IIS.
