Our org uses sharepoint as a document repository. The workflow for maintaining, updating and reviewing the document needs to be automated in sharepoint. We are looking for a solution with following features: 

-A distribution list should be configured for each document identifying the Reviewers, Approvers, and Informed individuals. 
-The document should me sent as a link to the individuals in the distribution list when the author is done with his changes.
-The individuals should see the documents submitted for review in a list of Actionables and should be able to mark the document as Reviewed or Approved or some other status. They should be able to provide review comments by filling out a form.
-The document version stored as a DocProperty inside the document should be updated every time the document is updated.
-The version number of the document maintained in sharepoint should match the version number inside the document.
-Sharepoint needs to maintain a list of related documents that are referenced from each document.
-The documents need to be baselined and labelled with a naming convention for the labels.

The workflow as described above should be flexible to be changed in the future. We also plan to migrate to sharepoint online in the near future.

What are the out-of-box or customizable solutions for this.

SharePoint designer workflow contains OOB action like ‘Task’(Reviewed or Approved action)/Email and list actions for your requirement.
