I've migrated a site/workflow/content types to a new site collection, and everything is working properly except the comments in approval tasks that are suppose to be logging to the history.  I'm requiring comments and a project number to be input into an approval task upon completion.  These values then get recorded to the log and spit out into the workflow history.  Below is the workflow and an example of what the history would look like, w/o the comments filled in. I'm not familiar with the index, so that might be where I'm messing up.  Any ideas?
