I have databases in a premium pool (250DTU) and I have noticed that there are times where the CPU usage is high 90-100% but the DTU usage does not come close to the limit. Can some explain what is going on here? Shouldn't the CPU be sized by Azure to accommodate the necessary DTU's?

DTU is a cauldren of disk, CPU and memory.  So your CPU being high does not mean that the DTU amount will also be high.  If your queries are CPU intensive but low on disk and RAM then that would make perfect sense.

If your DB activity is more CPU intensive than on  disk or memory then you should probably look at a vCore offering rather than DTU.
