However, if I have a second project that is a follow-up to another, how can break the connection between the first project and its site and then link the site to the second project and reuse? And would the issues and risks from the first project be maintained even though the links are broken.

Unfortunately  you can not associate one project site to another due to project specific GUID are associated with Project site, especially Issues and risks.

I would create second project with different project sites and add site link using site navigation option referring to each other.
