In my SharePoint site 2016 Top navigation (ribbon with user information) suddenly disappeared unexpectedly and site settings access from url is denied for home owners.
顶级导航也从中央管理中消失。
Top navigation disappeared from central administration too.

Would like to confirm if you are using custom master page, whether it is having any problem. If yes, you may swith the master page to the OOTB master page and try again. If no, maybe you could check the master page to see if the site action cotrol was removed from master page.
