So I am trying to estimate the cost of lifting our entire infrastructure to Azure and I am confused about the pricing of Azure SQL Database's business critical managed instances.

In this documentation: https://docs.microsoft.com/en-us/azure/sql-database/sql-database-high-availability It appears that the business critical tier includes 1 primary replica and 3 secondary replicas.

When using the Azure pricing calculator, would the "1" instance price include the 1 primary and 3 secondary replicas, or do I need to increase the instance count to 4 to obtain that kind of configuration?

The secondary replicas in the same data center/region are for free you only pay for the primary. However, if configure geo-replication to have secondary replicas on other regions, then in that case you have to pay for each replica located on a different region than the primary database.
