I have 2 separate lists, one called 425 and one called SF425. I am using the 425 field to store the name for the lookup in the SF425 table. If the Name Field matches the SF425 Award Field, I would like to set the 425 attachment name to the below.

Assuming that there would be a 1-to-1 relationship, then start by creating a workflow. Used a 2010, not a 2013.

There is an action that allows you to retrieve values from another list, based on a value in the current list. So, it appears that you would need to return LastName, FirstName, and DateCreated from the Lookup Table SF425. I don't have SPD in front of me, but I'm fairly certain you'll have to return each of those separately, and store the values into a Workflow variable.  Once you have the 3 variables populated, then you can easily update the field in the 425 list.
