Hi. this issue has being bugging for a few months and i cant seems to find the root cause for this.

i have a document library and folders in this document library are created programmatically via C# code. after folder creating is done, the code will break the permission and programmatically add permission for some users. this code has being working all alone. then suddenly users notify me that they do not have access to some folders which they are suppose to have. before this issue occur the users have went into the folder to read the files inside them or upload files. when i went in to check, i found that the permission to those folders got cleared out.

the sharepoint log doesn't seems to provide an insight on this. the modified date and modified by also seems to be incorrect. as based on the modified date is same as the creation date. which means to say these 2 columns, modified and modified by, did not capture when the permission has being remove and by who.
