Recently we had an issue occur in our SharePoint 2010 environment where multiple individuals received approximately two years of alert emails from this one calendar on this one sub-site. Not only did this occur but of these alerts that were sent out they were all said to have been modified on that date by an individual who was not even in the office at the time of this event. If you look in the calendar and view any of the events prior to this issue they all say they were modified by this individual.

I have pulled audit reports on the site and have viewed the event logs on all three servers however I am not sure what I am looking for. Out of the errors that I have found none of them seem to correlate with my problem and/or occur at the same time.

The environment is SharePoint 2010 running on three servers, one front end, one application, one SQL all running server 2008 R2. Recent changes to note, the systems team incorporated old windows updates the day prior of the event on all servers. By old I mean about four years worth. I don't know if this has anything to do with it, just thought I would mention it.

My bet is that it is directly related to the updates. There's a couple of ways that this could happen, if the updates fixed something that was malfunctioning in the alerts timer job then that could unjam months to years of alerts. Or if the update modified the list and the way that it stored data it could re-trigger the alerts process some how.

That all the items in the list say that they were updated by the same user is fairly telling. Unfortunately proving this will be difficult as it's possible to make changes to things that overwrite or do not get logged in the version history list.
