We are planning to use TFS as helpdesk and were wondering if there is an extension or add-on that can cover the functionality of helpdesk not natively in TFS. To be specific

Providing escalation / approval workflow capabilities.

I would consider not to use TFS as the primary helpdesk system. It is not designed for that. Instead I would look into a support system that can integrate to TFS, so only code bugs are created in TFS. e.g. Zendesk (we used that at my previous place, but otherwise I have nothing to do with the Company)
