We deployed a VM based on a the following image {"imagePublisher": "MicrosoftSQLServer", "imageOffer":  "SQL2017-WS2016"}. We deployed the machine using ARM and deployed the extension Microsoft.SqlServer.Management.SqlIaaSAgent. However, the SQL Server configuration option did not show up in Azure portal for this machine. 

It seems to be that actually the name of the extension is leading for this, and not the presence of an extension of a certain type. So renaming the extension from "SqlServerIaaS" to "SqlIaasExtension" seemed to do the job. Should this kind of functionality not be independent of the naming?

Not sure I fully understand the issue. 

The SqlIaasExtension is the one that adds the functionality to the portal. So that is the correct extension to install to get those features. 

Generally if you deploy a SQL image from the marketplace that extension will be automatically provisioned for you as well.
