Based on my knowledge, the Microsoft Equation Editor 3.0 isn't supported in Office 2016 and Visio 2016. To insert equation in Visio, it is suggested to insert equation in Word 2016 with build-in equation feature, then copy and paste text to Visio 2016 as a workaround.
