We migrated our environment from SP2010 to SP2013. WFs were migrated as is. That was done few years ago.

Now we have problems with one out of box WF which uses Approval template from SP2010.

Sometimes WF  does not create new tasks for users when previous tasks completed. This issue affects only libraries which start WF automatically once new item added.

According to the WF settings 3 users need to approve: user1, user2 and user3.

When WF started task for user1 created. Than I see in the WF tasks list, that user1 completed task. When I open WF instance for the file I see in the Tasks section, that task assigned to the user1 has status completed, but in the WF history section I do not see this event. The latest event in the WF history is: "task for user1 created".

I checked WF history list, it contains only records that task created, but does not contain records that task completed.

Because of this tasks fro user2 and user3 not created.

User1 has modify permissions on the WF task list and WF history list.

Obvious solution is to recreate WF, but unfortunately in this case that's not possible.
