I have a problem with wrongly tagged documents in SP2013. I will use example to explain it:

Let's say than I have a Country column name and State columns. document 1 is tagged Country:USA, State: VA

PowerShell would be your best option.  You can identify the documents where Country:Columbia and then change that to Country:XX.

To prevent further errors are you going to have a term for Country and a term for state (not hierarchical?)  If it happened once it will happen again.
